Frequently Asked Questions

We care for individuals that live at one of our partner communities and who are considered homebound.  This means that they have significant difficulty going out for medical appointments due to chronic health problems, advanced age, diminished mobility, and/or memory impairment.  We provide house calls when it becomes too challenging for someone to leave their home to see their medical provider. Our nurse practitioners act as primary care providers (PCPs). We have years of experience managing our client’s overall medical care. We are accepting new patients at the communities we partner with. If you are interested in working with Senior Care Northwest as your primary care provider (PCP) please contact our office at the phone number on this page. We are always open to discussing partnership with new communities to offer our services to their residents. If you are a community director, please contact us via email or phone call.

Nurse practitioners have independent practice in Oregon and our providers are licensed and trained in primary care and geriatrics. We can diagnose and manage common illnesses and medical problems, order and interpret diagnostic tests and labs, and prescribe medications and other treatments.  We will bring the primary care to her home.  Additionally, we have training and experience in caring for patients and families affected by Alzheimer’s disease and other forms of dementia.

We accept most insurance policies and bill insurance just like any provider’s office in the community.  There is no additional cost to the patient beyond the usual contracted rate for a provider house call visit. There may be a copay depending on your plan.  For a benefits check, please contact our office and our office manager will be happy to assist you.

The process is very straightforward. When you are ready to switch to a Senior Care Northwest provider, please inform your insurance carrier and your current doctor. Complete our enrolment forms and turn them in with a copy of any insurance cards you have. We will contact your previous provider for medical records.

Absolutely.  We collaborate with many physician specialists in the community.  You are welcome to take him to see his current specialists.
