Who We Are

Senior Care Northwest is an independent, locally owned and operated primary care practice founded on a simple idea: that older adults deserve high quality, accessible, and personalized healthcare delivered with compassion and presence. Since 2013, we’ve been bringing medical care directly to where people live, making house calls to senior living communities across the Portland metro area, the Willamette Valley, the Columbia River Gorge, and the Oregon coast.

We specialize in caring for home-bound older adults living in:

  • Assisted Living Communities

  • Memory Care Communities

  • Independent Living Communities

  • Senior Apartment Homes

  • Skilled Nursing Facilities

  • Adult Foster Homes

Our team is made up of experienced clinical and administrative staff who are passionate about working with older adults and take the time to truly get to know each patient. With deep expertise in geriatrics, primary care, and palliative medicine, we’re able to care for complex medical needs while also focusing on what matters most to each individual whether that’s comfort, function, or simply stability.

We work closely with families, community staff, caregivers, and other members of the care team to ensure that our patients receive coordinated, thoughtful care centered on their values and goals. Simply put, we believe the best foundation for high quality healthcare is strong, trusting relationships.

At the heart of our practice is our Clinical Director and founder, Kara Cooper, ANP, who started Senior Care Northwest after seeing how many older adults’ healthcare needs were not being met. What began as a solo practice has now grown into a trusted care team serving multiple communities and many patients while still keeping our care model personal, grounded, and responsive.

We’re proud of the work we do and honored to be invited into the lives and homes of the people we care for. Whether we’re managing chronic conditions, offering support at the end of life, or simply helping someone feel heard and cared for we bring the same dedication and heart to every visit.
